How Do I Request A Second Opinion?
Obtaining a second opinion from Virtual Radiologic is simple — just follow the steps below:
Step 1. Download, print and complete the following Second Opinion Request Forms:
Step 2. Send your Second Opinion Request Forms, medical images (on CD or film), copy of original radiology report and payment to Virtual Radiologic. Specific mailing and payment instructions are provided on the request forms. Once we receive these forms, a Virtual Radiologic Second Opinion specialist will contact you directly to discuss and review your request.
Step 3. Your Second Opinion specialist will work with you and your referring physician to obtain additional required information or images.
Step 4. One of our subspecialty trained radiologists will interpret your study. Copies of the resulting report will be sent to you and your referring physician. Please note: all individuals requesting a second opinion must be under the care of a referring physician who must be identified on the Second Opinion Request Forms.
